An employee received a minor injury and did not receive treatment nor lose time from work; should we report this to EP?

Yes. Please complete and check the box marked “Notice Only” on the EP Injury Report Form and fax it to EP. We will report incidents that did not result in any treatment or lost time as a “Notice Only” to our insurance carrier. For Notice Only claims, your production will not be contacted by the insurance carrier for follow-up questions. Our carrier will maintain the information on file should the employee seek medical attention in the future.

Does Disability Insurance exist for non-California residents?

There are only five states that provide state disability programs: California, Hawaii, New Jersey, New York, Rhode Island, and the Commonwealth of Puerto Rico. Please contact EP at 877.4EP.VPDI (877.437.8734) for assistance in filing claims in these states.

How do I report a Workers’ Compensation claim?

Workers’ Compensations claims are reported by calling our toll free number: 800.955.HURT (4878) between 8:00 am and 6:00 pm Pacific, Monday through Friday, excluding holidays and weekends. We will complete the necessary forms and do all the required reporting. (NOTE: Exception, see item VIII CAL-OSHA.) Claim information may also be faxed to 818.559.3283, 24 hours a day.

How do you calculate Workers’ Compensation?

EP calculates Workers’ Compensation on all hours worked or guaranteed at the straight-time labor rate, not on gross payroll, which typically includes overtime pay. We also classify each employee by his/her job description with the appropriate Workers’ Compensation rate as set forth by each state.

How does the physician pre-designation process work for California claims?

Your employees can choose to pre-designate their personal physician provided certain criteria are met. If your employee elected to pre-designate their personal physician prior to the injury and EP is in receipt of the Employee Pre-designation Form, they may seek treatment with their personal provider as an alternative to a provider within the Medical Provider Network. Criteria must be met by the physician, including 1) the provider must be their regular physician who has treated them in the past, 2) the provider must maintain their medical records, and 3) the provider has agreed to the pre-designation.

My employee has been injured on the job. Do I need to complete any forms?

Yes. Please complete the EP Injury Report Form and fax it to EP at the number shown at the top of the form. If medical treatment is required, please complete the Injured Worker Notification Form , and have the employee present this to the medical facility.

Our employee was treated for an injury and now is receiving medical bills. Will the insurance company pay those bills?

The adjuster will review the medical billing and determine if the invoice can be paid. EP can assist by providing the adjuster with contact information and the Workers’ Compensation claim number.

What are the Workers’ Compensation benefits?

The benefit amounts are statutory, which means they are determined by state law. Because each state’s law is different, benefits vary by state. The adjuster will assist with any benefit-related questions your employee may have. Entertainment Partners can provide the claim number and the adjuster’s contact information.

What is Disability Insurance?

Disability Insurance provides partial wage replacement to eligible employees who are unable to work because of injury, illness, or pregnancy. Disability Insurance also includes Paid Family Leave benefits.

What is the process for reporting a Workers’ Compensation injury?

If your employee sustained an injury while at work, the production company should notify Entertainment Partners (EP) using the EP Injury Report Form. After we receive the EP Injury Report from the production office, we will verify the employee worked on the date of injury. Once confirmed, we will report the injury to our insurance company. The insurance company then will assign an adjuster and a claim number within approximately 24 hours. The Workers’ Comp adjuster will review and investigate the claim and perform a fact-based analysis to determine whether the injury is covered by Workers’ Comp. The adjuster may contact the production office for additional information. If the employee requires a referral to a specialist after the initial diagnosis, authorization must be obtained from the adjuster.

For a serious or life-threatening injury or illness:

Please call 911 and direct the employee to the nearest emergency center or hospital.

The top priority in any serious injury is arranging medical treatment for the injured worker

For non-emergency Workers’ Compensation claims:

You may select a medical provider from the listing posted on the insurance company’s website. This site allows for a search by zip code within a specified radius. You may also contact the insurance company at 877.802.5246.

Whom can I contact to request a Certificate of Insurance for Workers’ Compensation?

To request a Certificate of Insurance from EP: You may contact the Workers’ Compensation (WC) Team at 800.955.4878 or by email to request a Certificate of Insurance. Before EP can issue a Workers’ Comp certificate, the production company must provide EP with a Certificate of Liability Insurance, including EP, et al, as additional insured. To provide a Certificate of Insurance to EP: Please review the EP Insurance Requirements and Questionnaire , then fax or email your Certificate of Insurance to the WC Team at 818.559.3283 or by email. If your production will be working with wild animals, watercraft, aircraft, stunts, pyrotechnics, foreign locations, or other possibly hazardous activities, you will need to complete and send the questionnaire part of the EP Insurance Requirements and Questionnaire . We will review all requests and respond as soon as possible.

California Medical Provider Network Information Booklet

California law requires your employer to provide and pay for medical treatment if you are injured at work. Your employer has chosen to provide this medical care by using a workers’ compensation physician network called a Medical Provider Network (MPN). This MPN is administered by AIG Claims, Inc. This notification tells you what you need to know about the MPN program, and describes your rights in choosing medical care for work-related injuries and illnesses.
