W-2 & Other Tax Form Inquiries – I believe I received a W-2 for an incorrect state. How do I request a corrected W-2?

W-2 & Other Tax Form Inquiries – I have not received my W-2 or 1099 from Entertainment Partners. Can I get your FEIN?
No, per our legal counsel we cannot provide this information.
The information is provided on your year-end tax form.
If you do not wish to receive your W-2 electronically, please complete and submit our Crew & Talent Inquiries and Corrections form to request a copy of your W-2/1099 be mailed or emailed to you.
W-2 & Other Tax Form Inquiries – I never received payment for the work listed on my W-2. Can you cancel and reissue me a new check?
If you believe you are missing a check, you may review your payment history by accessing our employee portal to reconcile payments issued at my.ep.com.
Once you identify the payment you’re missing and it has been more than 10 business days from the time it was issued, you will need to complete one of the following:

If it has been more than 10 days since you last worked, please contact your production company first to confirm a check has been released.
The production company will contact EP to start the void & reissue process if needed.
If the production company has wrapped, please complete & submit our Crew & Talent Stop Pay/Reissue form (link to form).
If it has been more than 10 days since the check was mailed to you, please complete the Background Actors Stop Pay Reissue form.
Talents need to contact their union first to see if a payment has been logged and released to you and ensure your union has your most recent address on file.
If your union states, the check was mailed to you and you don’t receive it within 10 business days from the time they mailed it, then you will need to complete our Crew & Talent Stop Pay Reissue form.
W-2 & Other Tax Form Inquiries – I received a W-2 and the Retirement Plan box is marked, I don’t have a retirement plan with EP. Can you send me a corrected form?
No. In most cases this is not an error.
Employees paid through a union production, even if not a union member, who had employer contributions made on their behalf toward the applicable union’s pension plan will have this box marked.
If the employee is not able to take credit on their tax return for their personal IRA because this box is marked, EP can provide them with a “pension letter” indicating why the retirement plan box was marked.
The letter will include the union’s name, address & phone number where the contributions were sent to.
The employee will need to obtain a second letter from the applicable union where the contributions were sent to indicating their pension eligibility status and submit to the IRS.

W-2 & Other Tax Form Inquiries – I received a W-2 for my family member who has passed away. What documentation do I need to provide?
You must notify EP and the union of your family member’s death by providing copies of the following documentation:
1. Death certificate
2. Last Will and Testament; if an Estate, letters of testamentary or letters of administration
3. Name, address, and Social Security number of beneficiary; if an Estate, name and address of Executor of the Estate and Federal Tax ID# for the Estate

W-2 & Other Tax Form Inquiries – I worked in New York and the W-2 is showing more than what I earned in that state. How can I get it corrected?
Section 672 of the NY tax law requires that if at any time during the year, a non-resident employee performs services in New York State, the wages reported in Box 16 (state wages) must match the amount shown in Box 1 (wages, tips, other compensation), regardless of how much was actually earned in that state.
If you are filing as a non-resident, remember to complete the IT203 and IT203-B forms.
W-2 & Other Tax Form Inquiries – The Year-to-Date total on my last check stub does not match my W-2 total. How can I get that corrected?

All W-2/1099’s will go out in the mail no later than the January 31st deadline.
If your address is current with EP, please allow ample time to receive it in the mail.
If you registered to receive your W-2 electronically, you will receive an email notification when your W-2 becomes available in January to access, view and download your tax form.
W-2 & Other Tax Form Inquiries – Why am I getting a 1099 for my corporation?
It is EP’s policy to issue 1099-MISC forms to all Loan-Out Corporations, as we have a responsibility to inform you of earnings received.
